You are welcome to contact a C.O.A.C.H. Advocate to simply discuss your situation. Upon meeting with a C.O.A.C.H. Advocate suggestions will be made regarding what can be done to address your issues or concerns. No action will be taken unless requested by the:
- Client
- Family on the client's behalf (if the patient is unable to make the request)
Confidentiality is highly respected and maintained. The focus is on the best interest of the patient.
A C.O.A.C.H. Advocate can provide:
- Information about hospital policies and procedures
- Information concerning the client's rights and responsibilities
- A way for you to voice issues, complaints or concerns
- An explanation as to how we will respond to your concerns or complaint.
- Action on your behalf to review complaints, concerns, inquiries, or needs
- Arrangements for a meeting if required to address your concern with the appropriate providers or facilities
Contacting a C.O.A.C.H. Advocate
Compliments or complaints can be made by meeting with a C.O.A.C.H. Advocate or by contacting via mail, email, telephone or fax. You are not required to identify yourself, but it will easier to identify and address issues, concerns, complaints or compliments with a validation.
