Often times, people without healthcare put off going to the doctor even for preventative care such as childhood immunizations or routine check ups. They are most likely to be hospitalized for conditions that could have been avoided. They also do not have regular doctors or access to prescription medications. C.O.A.C.H. Contacting a C.O.A.C.H. Advocate for your health and wellness needs

You are welcome to contact a C.O.A.C.H. Advocate to simply discuss your situation. This can be done by:

  • Telephone
  • Fax
  • In writing, by letter or email
  • By appointment
  • Office

Upon meeting with a C.O.A.C.H. Advocate suggestions will be made regarding what can be done to address your issues or concerns. No action will be taken unless requested by the:

  • Client 
  • Family on the client's behalf (if the patient is unable to make the request)

Confidentiality is highly respected and maintained. The focus is on the best interest of the patient.

A C.O.A.C.H. Advocate can also provide:

  • Information about hospital policies and procedures
  • Information concerning the client's rights and responsibilities
  • A way for you to voice issues, complaints or concerns
  • An explanation as to how we will respond to your concerns or complaint.
  • Action on your behalf to review complaints, concerns, inquiries, or needs
